Davao Doctors College - Admission Requirements

HIGH SCHOOL GRADUATE
1. Entrance exam result
2. Form 138 (Report Card)
3. Certificate of Good Health issued by DDC school physician (with lab results from Davao Doctors Hospital)
  1. Hepatitis Screening for all Health-related courses
4. Certificate of Good Moral Character from the previous school
5. Birth certificate – on security paper from the National Statistics Office (Original and Photocopy)
6. Application for Admission with Pledge of Agreement
7. Three (3) copies 2 x 2 new photos (colored with plain white background). Photos should show applicant in decent attire with collar
8. A photocopy of marriage contract for married students only - on security paper from NSO
9. A photocopy of parent’s/guardian’s ID (please refer to list of valid ID's below)
TRANSFEREE/SECOND COURSER
1. Transcript of Records for evaluation/reference
2. Entrance exam result
3. Honorable Dismissal / Certificate of Transfer Credentials
4. Certificate of Good Health issued by DDC school physician (with lab results from Davao Doctors Hospital)
  1. Hepatitis Screening (for all Health-related courses)
5. Certificate of Good Moral Character from the previous school
6. Birth certificate – on security paper from the National Statistics Office (Original and Photocopy)

4. Three (3) copies recently taken colored 2x2 pictures
5. One (1) Certificate of Good Moral Character from the Principal or Guidance Counselor
6. Application for admission
7. Student Visa (9f)
8. Alien Certificate of Registration (ACR) from the Bureau of Immigration issued in Davao City (issued upon presentation of Student Visa)
9. Photocopy of passport pages where name, photo, birth date and entry stamp marked from the Bureau of Immigration appear

CRITERIA FOR ADMISSION

PRE-REQUISITE FOR ENROLLMENT
1. A prospective foreign student-applicant for the tertiary or college level should be a graduate of high school/secondary school or its equivalent
2. General Admission Requirements
  1. Must meet school's criteria on entrance examination
  2. Must undergo and pass the interview
  3. Must submit a certificate of good health issued by the school physician (with lab. results from Davao Doctors Hospital)
    1. Hepatitis screening (for all Health - related courses)
3. Documentary Requirements for Student Visa
  1. Original copy of the school's Notice of Acceptance (NOA)
  2. Original copy of the student's Personal History Statement (PHS)
  3. Proof of adequate support to cover expenses incidental to his/her studies
  4. Scholastic record authenticated by the Philippine Foreign Service Post
  5. Police Clearance Certificate issued by the National Police Authorities in the student's country of origin or residence duly authenticated by the Philippine Foreign Service Post
  6. Medical Health Certificate issued by an authorized medical practitioner in the student's country of origin including standard-size chest X-ray
  7. Photocopy of Passport pages where name, photo, birth date and birth place appear
  8. Eight (8) 2x2 pieces photos
